We’re dedicated to providing timely and individualized intensive HIV case management and treatment for people of African descent who are newly diagnosed with HIV, at risk of HIV, or affected by HIV.

A Community Focused Approach to HIV Care for You

We are here to provide you with:

Intensive HIV case management

Improved access to intensive HIV case management with timely access to a Nurse Practitioner, Registered Nurse, Social Worker and Infectious Disease Specialist.

Patient navigation and linkage to community supports

Connect with a systems navigator who can guide you through the healthcare system and help you access community supports.

HIV education and awareness

Learn more about HIV care and treatment so that you can make informed decisions about your health.

Access to support services

Get help with transportation, applying for assistance, and other wrap-around services that can support your overall health and wellbeing.

Peer-led support groups

Connect with others who are going through similar experiences and gain support from a community that understands.

Support to develop personal and coping skills

Build resilience and personal strength with the support of our caring team.

Eligibility

Adults who identify as Black, Caribbean, or African are eligible for the AYA Circle of Care project. We believe in the strength of our community and we are committed to serving you with respect and dignity.
